I wasn't able to use my gift certificate at Vera before it closed - what do I do?

 I am more than happy to issue a complete refund for the amount remaining on your gift certificate that was purchased through my old business, Vera Massage & Day Spa. 

How do I get my refund?

There is a form at the bottom of the page, which I will need you to fill out completely. Once completed, please email it back to me at:

 Once the completed refund request form is received, please allow 7-10 business days to process the information to verify the gift certificate, and mail the check with your full refund.
